2. COLLECTION OF YOUR INFORMATION
The data collected from you while you are using the Application is utilized solely for the management of the Code event, and your data remains in the Application only while a Code event is open. The data you input into the Application is removed from the Application when you close the Code event.
The information we may collect via the Application depends on the information you choose to provide, including:
Any data request within the Application is optional and is not required to use the Application. You may voluntarily provide your demographic and other personally identifiable information (such as your name and employee identification number) or the personally identifiable information for a patient, including the patient’s name, date of birth, sex, and patient identification, if applicable, when creating a Code event. You may also provide information related to the Code, including rhythm check and pulse information. After a Code has ended, you may choose to export the data to a Portable Document Format (PDF). The data is not retained in the Application after the Code has ended, nor is the data shared with any other system.
The third party online/mobile store may collect and use your geo-location information. We are not responsible for any of the data collected by any such third party.
3. USE OF YOUR INFORMATION
The information you provide in the Application is used solely to manage a Code event. Having accurate information about you permits us to provide you with a smooth, efficient, and customized experience.
4. DISCLOSURE OF YOUR INFORMATION
We do not share information we have collected about you or a patient. Your information remains in the Application while a Code is open. When a Code is closed, you may choose to export the data as a PDF and print or save the information for your use. Any disclosure of the information in the PDF is your decision, and Christiana Care Health Services, Inc. is not responsible for any accidental disclosure of your information by you.
5. SECURITY OF YOUR INFORMATION
We use administrative, technical, and physical security measures to help protect your personal information. While we have taken reasonable steps to protect the personal information you provide to us, please be aware that, despite our efforts, no security measures are perfect or impenetrable. Any information disclosed online is vulnerable to interception and misuse by unauthorized parties. Therefore, we cannot guarantee protection against unknown threats to personal information.
6. POLICY FOR CHILDREN
We do not knowingly solicit information from or market to children under the age of 13. If you become aware of any data we have collected from children under age 13, please contact us using the contact information provided below.
